TRO Retinal Detachment.

Dapat URGENT CASE dah!

Pn. R (ramai pulak pt name mula R harini, ken?), umo 72.

Known case DM, HPT & dyslipidaemia - on Rx & f/up under KK PD.

She C/O reduced vision over Lt eye over few months, but worsening over this 2 weeks time.

Associated with floaters & curtain falling.

On examination : Is able to see flashes of light only (even finger movement pon dah x nampak).

Fundoscopy : unable to visualize fundus due to mature cataract.

I straight away called MO Oncall Ophthal Clinic, HTJS - S/T Dr. Punitha.. & she asked patient to go stat (ASAP) to their clinic for further assessment. If RD - for admission.

Pt    : Makcik pegi esok boleh x, Dr?

Aku : Makcik,  esok klinik pakar tutup. Makcik pegi hari ni lah ye. Tadi datang naik bas ke orang hantar?

Pt    : Anak makcik hantar naik kereta.

Aku  : Kalau camtu boleh lah pegi ye. Tapi jangan sampai pukul 12pm...dah dekat nak tutup klinik. Kalau betul RD, boleh sebabkan buta tau, makcik..

Pesanan untuk semua...kalau mata tiba2 terus tak nampak atau nampak cahaya je...jangan lambat2 gi klinik ye..bahaya tau..

Actually, pagi-pagi ni aku dah dapat macam2 jenis pt.

1. Case 1 : An uncle complaint of having foul-smelling urine for few months. No dysuria, no haematuria, no fever, etc.
Aku : uncle, you ada makan petai ke ape2 food yang banyak bau?
Pt    : Aku suka makan petai. Memang selalu makan. Anak aku pon suke makan petai...".
Buat jelah UFEME kan? Result normal. So, tau ler jawapannye kan...:-)

2. Case 2 : En. R, umo 50,
Pt    : Dr, tangan saya sakitlah. Dari semalam...
Aku : Ada buat ape sebelum tu?
Pt    : Saya mesin rumput..
Aku : Takkan mesin rumput sampai sakit tangan?
Pt    : Sebab getaran, doktor...
Aku: Tak biasa mesin rumput ke?
Pt   : Ni first time doktor. Selalu pekerja saya yang buat..

* See...actually aku memang dah tau the root cause tapi aku nak pt sendiri bgtau tau sebab ape die dapat sakit cenggitu. Because kalau aku yang explain...pt akan keep asking. " Yeke Dr? Yeke Dr? " Eventhough dowrang dah tau jawapannye...Kalau kite pon...kite akan tanye camtu, kan? Because it just happen naturally..
