Dear Young Doctors.

Dear young doctors,

As time goes by, u may not see the same person who just follow your advise & instruction as the previous decades. Put in mind ..for real that we are now in techno era where information are broad & can be searched with just a click on your gadget. Don’t try to persuade patient to understand what u have been thought in your Medical School because they won’t understand …ever. Sometimes they pretend to understand ..but which actually they aren’t. They just do that because they are afraid of hurting your feeling (since u are a nice, soft-spoken young doctor…like me, before). U may feel that u need to explain regarding their illness (from A-Z) so that they are aware of what’s going on in their body & that they can avoid complications from their diseases. With experiences, u may find that not all of them need these. U are able to see this from their eyes, their talk & not to miss their body gestures.

 Dear young doctors,

U have faced the interval of 5 years or more of learning Medical.
U have passed the hardship & grab the scroll in your hand.
U know what to do..and what not to do.
But,
When it comes to working ..it is totally different.
Things may not be so simple.

When patient comes to you..
They come with hope..
U may think that they just have conjunctivitis ..but they are thinking of getting blind.
U may think that they just having difficult breathing because of blocked nose ..but they are thinking of lung infection.
And so on, and so forth.

Remember,
What’s in your mind are totally different than what in theirs.
Sometimes, these turn into arguments which actually can be handled if you can tackle what they are actually looking for.
U can just ask what are actually their concern falls on.
(You will be surprise when you did this. Try first…)

Another important thing is to challenge them & make them think.
For example, if they ask for antibiotics (antibiotics is often the issue) (Usually they will push you to give antibiotic for them for any reason which u think is not reasonable).
U can explain the need of antibiotic if u think the person need that (often – NOT)
Or you can straight away ask them, “What made u think that u need the antibiotics?” OR “Do you know the function of antibiotics?”
Trust me, you will get ridiculous answers.
(Sometimes they just kept silent.)

SOME PATIENTS learned from their own school-of-thought. They don’t even know what are the meaning of:
‘Impression’
‘Differential diagnosis’
‘Most likely diagnosis’
‘Suspect of ..’
All they know is - they want a DIAGNOSIS.

Please keep calm .. don’t get irritated, and don’t laugh.
Just do your job as what you are taught in your Medical School.

* After all, these are only a piece of sharing from my own experience.
* Am not trying to offend anybody. I just intend to share a simple message from deep of my heart.
 

Nicotine withdrawal.

     Hopefully korang dah baca topic ni ~ Smoking - is a disease. . Then only u guys know that nicotine is more addictive than heroine or cocain.
    
Nicotine withdrawal is the group of symptoms that occur upon the abrupt discontinuation or decrease in intake of nicotine.

The effects of nicotine withdrawal include :

- Irritability, frustration or anger
- Anxiety
- Restlessness or impatience
- Difficulty concentrating
- Insomnia/ sleep disturbance
- Headache
- Increased appetite or weight gain
- Dysphoric or depressed mood

Merokok dari pelbagai sudut.

Sudut agama
- Hisap rokok termasuk dalam konteks percubaan membunuh diri
- Oleh itu ….
Sebelum hisap rokok …. Bacalah syahadah
Orang lain ….. bacakan Innalillah

 Sudut keluarga
- Anda sebenarnya PEMBUNUH atau PENYEBAB/PENYEBAR kematian atau penyakit dalam family anda
- Jangan salahkan anak anda jika mereka juga merokok seperti anda

Sudut kerjaya
- Anda adalah antara orang yang melanggar undang-undang
- Imej teruk betul ….
Seperti curi-curi hisap rokok

Sudut pembaziran
- Perokok ni premium tinggi
- Bayaran hospital tinggi
(Mengikut polisi insurans)

 Sudut mental
- Otak makin lemah
- Pelupa … (letak rokok, lighter dll)

Sudut fizikal
- Kebanyakkannya kurus
- Gigi KUNING/ bertar
- Badan/ baju bau rokok

Sudut sexual
- Cepat lemah syahwat
- Hubungan tidak intim ….
- Pentingkan diri sendiri

Ada orang kata -:
->Ikut kajian BANGSA YAHUDI bahawa Orang Islam ni ………..
Tidak membaca apa yang tertulis
Kalau baca pun, mereka tidak faham
Kalau faham pun, mereka tidak buat

     Tobacco smoking is classified as a Mental & Behavioural Disorder under the WHO International Classification of Diseases 
(ICD-10) coded as F17.2

Tobacco users are nicotine addicts.

Why do people smoke?
To relax.
For the taste.
To fill the time.
" Something to do with my hands. "
But, for the most part, people continue to smoke
because they find it too uncomfortable to quit.
Why is it so difficult to quit smoking?
1. Physical dependence on nicotine.
2. Behavioural dependence.

Hari Berkabung Malaysia.

Berkabung :(
Sila jangan tertipu muka nampak flawless
kerana faktor pencahayaan & jarak dari cam.
     Seperti yang semua sedia maklum, 22/8/2014 merupakan Hari Berkabung Malaysia.
Sememangnya kita semua bersedih di atas kemalangan yang menimpa penumpang-penumpang & krew-krew MH17. Walaubagaimanapun, sebagai umat Islam diharap ini dilakukan secara sederhana dan bukannya secara berlebih-lebihan.
     Innalillahhiwainnailaihirajiu'un pada mangsa yang beragama Islam ,serta takziah kepada keluarga mangsa :(

Depression.

     Ramai yang selalu tertukar antara stress & depressed.
Okey maksud stress dah pun aku explain dalam entri ni ~ Kenali & Tangani Stress. So, sekarang aku nak explain apa itu depression a.k.a kemurungan pula.

Major Depressive Episode
Ini adalah reference for
Diagnostic Criteria. 
Five (or more) of the following symptoms have been present during the same 2 weeks period & represent a change from previous functioning; at least 1 of the symptoms is either depressed mood OR loss of interest/ pleasure.
1. Depressed mood most of the day.
2. Markedly diminished interest/ pleasure in all/ almost all activities,
most of the day.
3. Significant weight loss (when not dieting) OR weight gain,
OR
Decrease OR increase in appetite.
4. Insomnia OR hypersomnia.
5. Psychomotor agitation OR retardation.
6. Fatigue or loss of energy.
7. Feeling of worthlessness OR excessive/inappropriate guilt.
8. Diminished ability to think/ concentrate, OR indecisiveness.
9. Recurrent thoughts of death,
recurrent suicidal ideation without a specific plan, OR
a suicide attempt, OR
a specific plan for committing suicide.
** And, any of these symptoms occur NEARLY EVERY DAY.

Kenali & tangani stress.

Definisi:

- Stress ialah tindak balas fizikal, emosi & mental kepada perubahan.
- Stress merangkumi tekanan, beban, konflik, keletihan, ketegangan, panic, perasaan gemuruh, kebimbangan, kemurungan & hilang daya yang biasa berlaku apabila berlakunya perubahan pada tubuh badan atau fikiran sendiri.

Kenapa anda stress?

Diri sendiri – kecewa
Keluarga – masalah rumahtangga, kewangan, anak dsb
Pekerjaan – tidak cemerlang dalam kerja, tidak naik pangkat, dibuang kerja, kena tukar dsb
Komuniti – bertengkar dengan jiran
Kesesakan lalulintas
Persekitaran yang tidak selesa
Tragedi
Menunggu lama

Sukar tidur
Sakit kepala

Merokok berlebihan
Makan berlebihan
Menyalahguna dadah

Mudah lupa
Takut gagal
Rendah diri

 Apa yang harus dilakukan apabila anda marah?

Bertenang…Tarik nafas & lepas perlahan-lahan
Kenal pasti punca anda marah
Mendapat nasihat orang lain
Beredar ke tempat lain bagi mengawal kemarahan
Berbincang dengan ahli keluarga atau rakan
Membiasakan diri memaafkan & menerima kenyataan bahawa manusia juga melakukan kesalahan
